DCB whitelist shortcode for user association

This is to white list short code for DCB user
association for 18Q1. We will only whitelist
code that will actually trigger the premium SMS
warnings. Namely
CL - 9240
NO - 2171
BR - 4141, 8000
FI - 17110
ID - 363

There's also a config update for Thailand (TH).
