Memory mapped coverage (take 2) 

Add a build variable, CLANG_COVERAGE_CONTINUOUS_MODE, instead of
selecting based on paths.

Test: CLANG_COVERAGE_CONTINUOUS_MODE=true m and verify continuous mode works
