Re-applies the Android “communication device” route (this controls both output + input mic routing) whenever the call audio device changes — helps prevent the USB-C headset mic from being lost when the screen turns off/locks.

Improves audio-focus change handling so that after a focus blip (lock sound, etc.) Conversations re-asserts MODE_IN_COMMUNICATION, unmutes mic, and re-checks routing.
