msm: kgsl: Always do an OOB for DCVS

The GMU requires that an OOB for DCVS occur both
during warm boot and cold boot in order to set
the GPU clock frequency. This fixes a bug where
the GPU would be stuck running at the minimal frequency,
causing DCVS to change the power level to TURBO_L1
due to a large busy counter value.

Also, change the busy mask for DCVS to the proper
value.
